DOT Physical Exam

We know how important the DOT Physical Exam is for you. Let's keep you on the road!

Below is a list of items you should bring to your exam:

  • If you wear glasses or contact lenses, bring them with you.
  • If you wear hearing aids, bring them with you.
  • If you have high blood pressure, not a problem if it is controlled.
  • If you are using insulin for diabetes, please have your treating provider complete the Insulin-Treated Diabetes Mellitus Assessment Form, MCSA-5870, and bring it with you within 45 days of completion.
  • If you take medications, bring them with you or a detailed list including each medication name, dosage, and frequency.
  • If you have prior certificates of exemption, bring them with you.

What to expect during your DOT physical exam:

  • You will answer a few questions about your medical history.
  • Include any medications you are currently taking.
  • The exam will include vision, hearing, blood pressure and a basic urinalysis test.
  • The medical provider will review your medical history with you and perform a physical
  • After the medical history and physical exam are completed, we will get your paperwork ready for you.
  • You will be provided with a copy of your “Medical Examiner’s Certificate” and a second copy for you to provide to your employer.
  • We will file a copy of the “Medical Examiner’s Certificate” and “Medical Examination Report Form” at our office.
  • We will process your paperwork with FMCSA, and you should receive your DOT medical card, which you should carry with you every time you operate a commercial vehicle.
How long is a “Medical Examiner’s Certificate” valid?

DOT certifications can be given for as little as 3 months and as much as 2 years. The certification length depends on your health.

If you use glasses or contacts bring them with you so you can use them during your vision test. We are looking for a visual acuity of at least 20/40 in each eye with or without corrective lenses.
Having high blood pressure will not disqualify you, but it does need to be control with medication.
If you wear hearing aids bring them with you, so you can use them during your hearing test.
We are not performing a drug test. We are testing for specific gravity, protein, blood, and sugar. This will give us an indication of other possible health conditions.
